Graduation Verification, Transcript and Student Records Request Forms

Greetings Clark Alumni,

Clark Montessori is happy to provide student transcripts to Clark Alumni with the paid fee and the below request form completed and remitted.

The completed from may be remitted in two ways:

  -  mail to: Clark Montessori

The fee schedule is as follows:

All of the above is "per request". For example, if two student verifications are requested, the total would be $4.00.
